The Health New Zealand Te Whatu Ora Waikato My Advance Preference Statements (MAPS) connects to the paramount Waikato awa/river that is the mauri/lifeforce of the region and taonga/treasure to the people of Tainui waka.
Mental health journeys can be likened to the Waikato awa/river, which is ever-changing. Around every bend are tranquil currents that gently propel us forward or whirlpools and rapids that require extra support to navigate. MAPS can be a useful tool to help us navigate the whirlpools and currents we may encounter on mental health journeys.
Information on MAPS
MAPS is a template that can be your voice in treatment and care decision making, at any time or anywhere on your journey with mental health & addictions services. Your MAPS can share your voice with kaimahi/staff, whānau/family and support networks to understand who you are and what treatment preferences you would like respected during your time with the services, in the community, hospital or wherever you choose.
This page is for tāngata whaiora, support people and kaimahi/staff in the Waikato district who want to learn more about creating My Advance Preference Statement (MAPS). Here, you can find resources to help you write your own MAPS, for services promoting the use of MAPS, for staff on the process of supporting tāngata whaiora to develop a MAPS and contacts for further support.
MAPS templates
The MAPS template has an accompanying fact sheet to help you understand more about what you should include in a MAPS and how you can go about creating it. You can download these templates onto your computer from here. The forms can be filled out by typing directly into them, or printed. You can request support from Waikato Health New Zealand mental health and addictions staff to access or print this template.
After completing your MAPS, you can submit them to your local MAPS contact. Don't forget to save your MAPS as you work on it and make sure you keep a copy of your MAPS on your computer.
How to complete and submit your MAPS

Download a Waikato MAPS or ask a mental health staff member to print a copy for you to complete.

Complete your MAPS digitally or manually. You can do it yourself or with the support of trusted whānau or staff who can discuss some options. This MAPS is personal to you and what you believe supports your wellbeing.

Make sure your personal details are correct. If you have a key worker, make sure to include their details and the details of your care team. Decide whether you would like to participate in the Te Kete Rongomau research project and opt in or out of being contact by this group.

Email your MAPS to TeKeteRongomau@waikatodhb.health.nz or hand it to your key worker. Your MAPS will then be loaded onto your clinical record so it can be accessed by mental health staff at any time. We also encourage you to share your MAPS with a trusted support person, whānau or staff member.
Get support
You can write your own MAPS or choose to have someone you trust help you, such as your whānau, friends, or support worker. The Lived Experience advisor is the key contact for the Health New Zealand Te Whatu Ora Waikato MAPS project.
