LAKES MAPS

Mental Health Advance Preference Statements (MAPS) are a way for you to be involved in decisions and tell people what you would like to happen in your mental health care and treatment.

 

This page is for service users, and support people in the Lakes district who want to learn more about creating Mental Health Advance Preference Statements (MAPS). Here, you can find resources to help you write your own MAPS and contacts for support around MAPS.

Information on MAPS

The Health New Zealand Te Whatu Ora Lakes Mental Health Advance Preference Statements (MAPS) is based on Te Ara Tauwhirotanga: Pathways that Lead Us to Act with Kindness.

Te Ara Tauwhirotanga is the model of care adopted by Health New Zealand Te Whatu Ora Lakes. It is organised under three pou: Tangata (People), Tikanga (Values), and Taiao (Environment). Under the three pou are ten principles that explore what it means to "Act with Kindness."


We have created booklets to help you to write your MAPS. They explain MAPS and provide examples of what you can include in each section. MAPS are your will and preferences, but they must also be realistic for these to be supported.

MAPS templates

We have created helpful templates to help you in writing your MAPS. You can download these templates onto your computer from here. The forms can be filled out by typing directly into them. After completing your MAPS, you can submit them to your local MAPS contact. Don't forget to save your MAPS as you work on it and make sure you keep a copy of your MAPS on your computer.

Remember to save your MAPS every time you make changes.

Get support

You can write your own MAPS or choose to have someone you trust help you, such as your whānau, friends, or support worker.

If you are a service user who is known to the Lakes Mental Health &  Addiction Services, your MAPS will be uploaded into our client record system.  We can then easily access your MAPS,  if needed.
